TN History for Kids booklet order form

Booklets cost $2 each, plus a Shipping and Handling (S&H) charge equal to the following:

* $3 S&H per booklet for 2 booklets  (minimum order is 2 booklets; minimum check accepted is $10)
* $2 S&H per booklet for 3-5 booklets
* $10 TOTAL S&H for 6-15 booklets
* $15 TOTAL S&H for 16-25 booklets
* $20 TOTAL S&H for more than 26 booklets

Currently we ONLY ACCEPT CHECKS. To order booklets, please print this form and send it, along with a check for the correct amount made out to Tennessee History for Kids, to the address given at the bottom.
