To order booklets, fill out the on-line order below, PRINT it out and send it to us.

IMPORTANT:

You must fill in all fields on the form, then click on the green “Print Order” button. IT WILL TAKE ABOUT 30 SECONDS FOR YOUR ORDER TO PROCESS…please be patient during this time… don’t hit any buttons or give up! A PDF of your form will be generated and you will be directed to a page where you can download the form by clicking on the green “Download PDF” button.

Booklets cost $2.50 each, plus Shipping and Handling (S&H)

Shipping and handling charges will be calculated for you automatically when you fill out the on-line form.

Please note:

• We cannot process orders until we receive a purchase order or a check.
• We do not accept credit cards.
• We only accept purchase orders of $50 or more.
• Minimum order is 3 booklets; minimum check accepted is $12.
